First Appointment at Mahogany Home Clinic

Online Booking Platform – Jane

After booking your appointment you should have received a welcome email to Jane, our online booking platform. Follow the instructions to set up your account. Here you are able to change or book appointments on your own as needed. 

Physiotherapy Intake Form

You should have also received an intake form to be filled out prior to our first session if possible. There is a spot to list previous injuries as well as to input your credit card info so I can complete the billing after our session. All information is secure and will be kept confidential. 

Location

The general location is Masters Square in Mahogany. The exact location will be sent to you after booking along with your confirmation.

What to Bring: 

  • Comfortable shorts 
  • T-shirt, tank top, or comfortable bra to be able to get to shoulder blade/upper back if needed
  • Indoor shoes (optional), please bring running shoes for any walking or running analysis on the treadmill

Parking

When arriving to your physiotherapy appointment at the Mahogany Home Clinic please park directly in front of Mike’s place or on the other side of the street. Please leave the spots in front of the neighbour’s houses for the neighbours.

When to Come

Please knock on the front door as close to your appointment time as possible. Please do not arrive early as Mike may be finishing up with the previous patient. There is a short time gap between each patient in order to prevent anyone waiting at the door.

Welcome Committee

Mike’s cockapoo Vida will likely be greeting you at the door and may hang out for moral support at the beginning of your session. If you do not like dogs or have any allergies simply contact Mike prior to your appointment and Vida will be given some nap time.
