For Health Care Practitioners

Are you a health care practitioner or in the healthcare industry and want to work together? Don’t hesitate to reach out and we can set up a collaboration meeting to get to know one another. This can happen over the phone or better yet in person at the home clinic in Mahogany so Mike can explain in more detail what we are trying to achieve and how you can be a part of it. The goal is to find win wins with other practitioners so we can help support your patients in whichever way you deem fit.

Trust

Trust is a huge part of any collaboration. At Inspire Health & Performance we have trust issues and therefore work very closely with only a select few so that we always know our patients will be in good hands.

Sound like you? I’m sure you feel the same way and therefore we have established a referral system that doesn’t steal patients from you so you’re not left in the dark wondering how your patient did. We have a referral system in place that keeps you informed while maintaining the referring practitioner as the primary care provider and us as a supporting role in other areas the patient may benefit from.

For example, if you are physiotherapist referring to us, we will not recommend to your patient they should come see us for physiotherapy. You remain the primary physiotherapy and we will strive to fill in other deficits in the patient’s big picture such as sleep, nutrition, and mental health through our health coaching service or through a direct referral to one our practitioners in those areas.

Referral Process

If you feel we may be able to support one of your patients please see below on steps on the referral process. Our goal is to make things quick and easy for you and for us.

Please Include: 

  • Heading: What the referral is for – e.g. Health Coaching Referral
  • Referring practitioner name 
  • Patient first name and last initial (pending patient consent) 
  • Reason for referral (Examples: assess and treat; patient may benefit from further support with sleep to support current rehab program)
  • If you would like to set up a time to chat to discuss in more detail please request this in the referral

Summary of Health Coaching for you and your team: 

Health Coaching – Inspire Health & Performance 

A local physiotherapist Mike has introduced a health coaching service as a way to help practitioners deal with big picture concerns that may fall outside of our scope of practice. These 1 on 1 coaching calls are often every 2 weeks to start and will involve an in depth intake form (made in collaboration with a dietitian and psychologist) gathering information on the patient’s sleep, movement, nutrition, and mental health. The coaching calls with then identify the patient’s top level goals, discuss and prioritize barriers towards them, and create a very specific action plan related to the most important steps forward. The coaching calls deal with the big picture while a screening/referral process is in place to get the patient professional healthcare support (e.g. psychologist, dietitian or sleep centre) when necessary. Any referring practitioner remains the primary practitioner and the health coaching calls will be there to only support the current program in place. Communication will also be included back to the referring practitioner in the form of an email update monthly. The idea is that the coaching calls supplement and not replace the current program in place with any referring practitioner.
